In short
For a fee you do not understand, say Je ne comprends pas ces frais. À quoi correspondent-ils ? For a payment you do not recognise, use Je ne reconnais pas cette opération. Give its date, amount and statement description, then ask for an explanation or review.

1 · Identify the line on your statement
Le libellé is the description shown beside the transaction. Débité means taken from the account.
2 · Ask what a fee covers
Des frais is plural. Refer back to it with ils or les.
3 · Describe an unrecognised payment
This is a different query: you do not recognise the payment itself. Name it clearly before asking about the procedure.
4 · Ask for a review and track the reply
Examiner une demande means to review a request. Asking for a refund states what you want.
