What does tout en mean in French?

Elle répond tout en souriant. She replies while smiling.

Focus: two actions happening together

She answers my question while smiling.

Although I agree, I am asking for clarification.

Focus: a fact followed by a qualification

Although I agree in principle, I am asking for clarification.

Smiling naturally accompanies a reply. Agreement might lead someone to expect an immediate yes; asking for details adds a reservation. In that second sentence, “although” brings out the contrast. English “while” can also express this kind of qualification.

A proposal, an acknowledgement and a refusal

Une proposition de groupe
  1. Nora

    Tu apprécies notre projet ?

    Do you like our project?

  2. Sami

    Oui, je le trouve intéressant.

    Yes, I find it interesting.

  3. Nora

    Alors, tu vas participer ?

So, are you going to take part?

  4. Sami

Although I like the project, I can’t take part. I’m short of time.

Sami accepts the positive point and then limits the conclusion Nora draws from it. He likes the project; he has too little time to join. This is a concession: both statements stand, and their relationship creates the contrast.

Elles discutent tout en se préparant.

They chat while getting ready.

The women perform both actions. Tout and préparant keep their spelling; se stays before the participle.

Although Léa likes the project, Sami refuses to take part.

Here the two ideas have different actors. A full clause names Léa and Sami clearly.
